What you will do:

• Facilitate safe and timely hospital discharges, aligning with D2A pathways and local discharge policies.

• Conduct Care Act assessments and Mental Capacity Act assessments, and contribute to Best Interest decision-making when required.

• Prepare person-centred discharge plans, coordinating essential community support, reablement, or short-term placements.

• Collaborate with patients, families, and multidisciplinary team colleagues to ensure clear and effective discharge planning.

• Identify and manage any safeguarding concerns in line with statutory responsibilities.

• Maintain accurate and timely case records while managing a dynamic caseload.

• Provide supervision to a team member, upholding high performance standards and operational compliance.

• Participate in the Duty Manager rota, managing work allocation and triaging referrals effectively.

Essential Requirements:

• Qualified Social Worker with registration with Social Work England.

• Experience in hospital discharge, reablement, or adult social care.

• Strong understanding of D2A pathways, safeguarding procedures, and relevant legislation.

• Ability to work autonomously and effectively within a multidisciplinary environment.
