Exciting opportunity for an Adults Social Worker within Hospital Discharge in Leeds, earning £35 per hour for a 3-months locum contract. What you will do:
- Facilitate safe and timely discharges from the hospital by working closely with patients, their families, and the multidisciplinary team.
- Conduct detailed assessments of patients' needs, creating effective care plans that ensure ongoing support outside the hospital setting.
- Collaborate with various community resources to coordinate services and support for patients post-discharge.
- Maintain accurate and timely records in adherence to Social Work England standards to ensure the best outcomes for clients.
- Advocate on behalf of clients, ensuring they receive all available support and resources to promote their independence.

Requirements:
- Possession of a degree or equivalent qualification in social work.
- Current registration with Social Work England is essential.
- Substantial frontline experience within UK Social Services as a qualified Social Worker.
- Strong communication and interpersonal skills, with an ability to work as part of a multidisciplinary team.
