What you will do
- Conduct Care Act Assessments for individuals requiring ongoing care and support as part of the Home First pathway, ensuring every resident achieves maximum independence.
- Review care and support plans systematically to maintain a high standard of safety, wellbeing, and choice for each individual.
- Champion rehabilitation pathways including specialist and slow-stream rehabilitation protocols, as well as Collar and Brace Aftercare Services (CBACS).
- Work in partnership with Havering's Quality Assurance Team to address concerns in care homes or among care providers, ensuring the highest standards of care delivery.
- Support individuals in transitioning from rehabilitation to long-term care arrangements, striving to optimise their independence and quality of life.
