What you will do:
- Work across Pathway 1 (care at home) and Pathway 2 (care home beds) within the Intermediate Care service.
- Operate as an autonomous practitioner within an integrated multi-professional team.
- Manage patients on shared caseloads within community and care home settings.
- Undertake assessments of patients with complex conditions using specialist clinical reasoning.
- Identify patient needs, agree on goals, and provide tailored therapy interventions, including equipment and rehabilitation programmes.
- Maintain a holistic approach to therapeutic interventions, crossing professional lines as necessary.
- Involve patients and carers in all decision-making processes to keep patient care central.
- Offer support, supervision, and education to peers, junior staff, and students.
- Be accountable for tasks delegated to Therapy Assistant Practitioners and Integrated Care Workers.
- Prioritise patients based on clinical needs and manage task allocation effectively.
- Consistently deliver a high standard of care.
- Provide coverage and resilience to other locality teams as needed due to operational demands.
