What You Will Do:
- Conduct comprehensive assessments to identify the needs of adults
- Develop and coordinate tailored care and support plans
- Work collaboratively with external agencies and community services
- Maintain accurate and compliant case records and reports
- Provide emotional support and guidance to service users
- Lead and participate in safeguarding investigations to protect vulnerable adults
Requirements:
- Qualified Social Worker with Social Work England registration
- Experience within adult social care settings
- Strong assessment, safeguarding, and care planning skills
- Excellent communication and organisational abilities
