Additionally, support individuals through the Community Independence and Wellbeing Team, offering community social work and occupational therapy services for those in various care and support environments. The Hospital Team plays a critical role in providing 7-day social work and social care support to help individuals and families during hospital discharges or within community teams for people with learning disabilities and mental health needs.
