What you will do:

Manage a diverse caseload across supported living services in and out of the Reading Borough.
Collaborate with the project Occupational Therapist, care providers, Housing Services, and health colleagues to ensure integrated support.
Engage in regular site visits and office work to maintain high-quality service delivery.
Develop and review personalised, strength-based support plans that focus on positive outcomes.
Implement innovative, tech-based solutions and reablement approaches to support independence.
Work closely with individuals, their families, and providers to ensure services are co-produced and align with the REACH standards.
