Social Worker – Adults Hospital Discharge Team
Location: West Lothian
Contract: Temporary, Full-time
Pay: £42 per hour

Seven Resourcing are looking for an experienced Social Worker to join the Adults Hospital Discharge Team in West Lothian. This is a full-time, temporary position offering £42 per hour.

In this role, you will support adults through the hospital discharge process, ensuring safe and timely transitions back into the community or appropriate care settings. You will work collaboratively with health professionals, families, and other agencies to develop and implement effective care plans that promote independence and wellbeing.

Key Responsibilities of the Social Worker

• Lead on hospital discharge planning, ensuring compliance with statutory requirements.

• Undertake assessments, care planning, and reviews in line with legislation and best practice.

• Work closely with health colleagues, families, and external agencies to ensure smooth transitions.

• Support adults to access appropriate community-based services and resources.

• Maintain accurate and timely case records.

• Manage a varied caseload effectively while prioritising urgent discharges.

Applicant Requirements

• We don't offer sponsorship, must be able to work full-time hours.

• Social Work qualification (BA/MA/BSc/MSc in Social Work, CQSW, or DipSW).

• Registration with Social Work England or equivalent body.

• Completed ASYE (Assessed and Supported Year in Employment).

• Minimum of 2 years’ post-qualifying experience within adult services.

• Experience of hospital discharge or transferrable skills from a relevant adult social care role.

• Enhanced DBS/PVG check.

• Full UK driving licence and access to a vehicle.
