Job Purpose

To deliver high-quality, person-centred social work services to adults with care and support needs, as well as their families and carers. You will work as part of a multi-disciplinary Adults Community Team, supporting independence, protecting vulnerable adults, and helping people achieve their personal goals in line with statutory duties and local policies.


Key Responsibilities

  • Conduct strengths-based assessments in accordance with the Care Act 2014, including risk and Mental Capacity Act assessments.

  • Co-produce personalised care and support plans that promote wellbeing, independence, and meet eligible needs.

  • Undertake safeguarding enquiries under Section 42 of the Care Act, ensuring robust responses to protect adults at risk.

  • Work in partnership with professionals from health, housing, and the voluntary sector to provide integrated, person-centred care.

  • Manage a varied caseload of complex cases with autonomy and accountability.

  • Act as an advocate for service users, ensuring their voice and preferences are heard and respected.

  • Maintain accurate and up-to-date case records using the electronic case management system.

  • Participate in supervision, team meetings, and continuous professional development in line with Social Work England requirements.

  • Contribute to service development and improvement initiatives.


Person Specification

Essential Criteria:

  • Qualified Social Worker with current registration with Scottish Social Services Council (SSSC).

  • Demonstrable experience in adult social care, ideally within a community setting.

  • Solid understanding of relevant legislation, including the Care Act 2014, Mental Capacity Act 2005, and the Human Rights Act 1998.

  • Ability to carry out assessments and manage risk effectively in complex scenarios.

  • Excellent communication and interpersonal skills, with the ability to build trusting relationships.

  • Experience of working collaboratively with service users, families, carers, and other professionals.

  • Strong organisational skills and the ability to work independently.

  • Proficient in IT and experienced in using electronic case recording systems.
