What You Will Do

• Lead Single-Handed Care Review projects, evaluating and optimising existing care packages.

• Carry out 10 single-handed assessments per week across a 15-week period.

• Recommend and provide equipment that reduces reliance on carers and promotes independence.

• Order and manage equipment needs in line with the Care Act (2014).

• Work collaboratively with the Adult Social Care team to reassess and refine support plans.

Requirements

• HCPC registration as a qualified Occupational Therapist.

• Demonstrable expertise in single-handed care reviews.

• Confident car driver, essential for community-based assessments.
