What You Will Do

• Carry out medicines reconciliation and clinical reviews to optimise patient care.

• Make appropriate clinical interventions to improve safety and treatment outcomes.

• Process patient discharges using electronic prescribing systems efficiently.

• Provide expert advice and support on general ward-based duties.

• Work closely with multidisciplinary teams in a dynamic acute hospital environment.

Requirements

• Registered with the Health and Care Professions Council (HCPC).

• Minimum of 2 years’ experience in an acute hospital setting.

• Car driver preferred for ease of travel within the area.
