What you will do:

- Provide professional social work services to adults, families, and carers in accordance with the Care Act 2014.
- Engage with individuals, families, groups, and communities to assess and intervene using professional judgment to promote choice, control, and independence.
- Demonstrate expert and effective practice in complex situations, managing higher levels of risk and liaising with a wide range of professionals.
- Assist individuals, carers, and families in developing support plans that meet identified outcomes using a strength and asset-based approach.
- Promote wellbeing and independence to help individuals and carers retain or regain skills and confidence, preventing or delaying deterioration.
- Facilitate effective engagement with families for contingency planning to manage complexity and changing circumstances.
- Ensure understanding of the NHS Continuing Healthcare process and support as needed.
- Ensure compliance with the Care Act 2014 and other relevant legislation while using advocacy services and carrying out reviews, including mental capacity assessments.
- Promote a learning culture and personal professional development through supervision and guidance to less experienced social workers.
- Take the role of Practice Educator (or train as one) for social work students and prepare as an ASYE assessor and supervisor for newly qualified social workers.
