Senior Social Worker – Older People’s Community Mental Health Team in Oldham

We are thrilled to present an exciting opportunity for a Senior Social Worker to join the Older People’s Community Mental Health Team in Oldham for a locum period of 3 months. What you will do:

  • Conduct comprehensive assessments for service users, carers, and families, evaluating support needs and developing appropriate Care & Support Plans.
  • Deliver, evaluate, and update care plans to resolve social care, health, and well-being challenges.
  • Collaborate with service users, carers, and families to support informed decision-making within the framework of self-directed care and individual budgets.
  • Proactively address safeguarding issues, adhering to relevant policies and procedures.
  • Manage risk assessments and contribute to positive behavioural changes to reduce risk.
  • Participate in crisis intervention, assessing urgency and determining appropriate legal interventions.
  • Maintain high data quality standards in social care systems and ensure confidentiality of personal information.
  • Attend and contribute to meetings, case conferences, and decision-making forums, and chair meetings as required.
  • Build and maintain relationships with partner agencies, including health and voluntary sector organisations.
