What you will do:

- Conduct comprehensive initial assessments for stroke patients and collaborate effectively with the multidisciplinary team (MDT).
- Manage and treat complex cases, utilising your insight into neurological therapy for optimal patient outcomes.
- Foster a deep understanding of the interplay between physical, cognitive, psychological, and social elements of illness, staying informed about relevant legislation.
- Execute precise Risk Assessments and contribute to patient-specific MDT meetings, including FIM/FAM, goal planning, and detailed report writing.
- Apply excellent communication skills to connect with patients, their families, and the broader healthcare team.

Requirements:

- HCPC registration is essential.
- Must be a car driver to facilitate patient home visits or off-site care settings.
- Experience with stroke, acute brain injuries, spinal rehabilitation, and prolonged disorders of consciousness is required.
