What you will do:

  • Conduct comprehensive mental health assessments, ensuring compliance with the Bailiwick of Guernsey mental health law and the Guernsey Capacity Law.
  • Promote independence and personalisation in care planning, enabling service users to manage their own needs and risks.
  • Lead multi-disciplinary team discussions and coordinate patient care plans with a focus on enablement and reablement opportunities.
  • Participate in the on-call duty roster to deliver timely mental health assessments and interventions.
  • Foster effective communication with patients, their families, and multi-disciplinary teams to provide coordinated and compassionate care.
  • Act as a health promoter and educator, helping individuals and their support networks to understand and navigate mental health issues.
  • Advocate for service users, ensuring their involvement and respect for their dignity in all aspects of care delivery.
  • Lead on service development initiatives, contributing to policy improvements and organisational objectives.
