What you will do:

- Deliver high-quality clinical pharmacy services within the multidisciplinary team, ensuring effective patient care.
- Support medication management by advising on the safe and effective use of medicines, reviewing patient prescriptions, and monitoring their effects.
- Collaborate with healthcare professionals to develop and implement patient care plans.
- Contribute to the development and implementation of pharmaceutical policies and procedures.

Requirements:

- Degree in Pharmacy
- 1 year pre-registration training and experience or equivalent
- GPhC registration
- Clinical Pharmacy Diploma or evidence of working towards this
- Evidence of continuing professional development
- Hospital experience
- HCPC registration required
- Car driver preferred, to enable you to travel across sites if necessary
