What you will do:

• Conduct comprehensive social care needs assessments to ensure that each adult receives the tailored support they need.
• Engage in support planning to develop practical and effective care plans that enhance the well-being of service users.
• Take charge of commissioning services, ensuring resources are allocated efficiently and effectively.
• Carry out risk assessments and manage risks adeptly, including attending Multi-Agency Risk Assessment Meetings (MARAMs).
• Be at the forefront of safeguarding, protecting adults and ensuring their safety and dignity are always upheld.
