Key Responsibilities:
Caseload Management
-
Act as Care Coordinator under the Care Programme Approach (CPA), leading care planning and monitoring recovery outcomes.
Clinical Assessment & Intervention
-
Conduct comprehensive biopsychosocial assessments and deliver personalised interventions in line with NICE guidelines.

Support clients in achieving stability, independence, and improved mental health.
Risk & Crisis Management
-
Perform risk assessments, formulate risk management plans, and respond to acute episodes, including safeguarding concerns and mental health crises.

Engage in home visits and urgent clinical interventions as required.
Multi-Disciplinary Collaboration

Work in partnership with psychiatrists, social workers, psychologists, and third-sector organisations to deliver integrated support.
Medication Oversight

Support medication adherence, administer depot injections, monitor side effects, and offer psychoeducation around pharmacological treatments.
Documentation & Compliance

Maintain accurate, timely records using the Trust’s electronic patient record system.

Participate in clinical audits, service improvement meetings, and reflective supervision.
Candidate Profile:
Essential Qualifications:
-
Registered Mental Health Nurse (RMN) with active NMC registration.
Core Experience & Skills:
-
Proven experience in adult community mental health or recovery-based services.

Strong clinical decision-making and risk assessment skills.

Familiarity with Care Act, Mental Capacity Act, Mental Health Act, and safeguarding legislation.

Capable of lone working in community and home settings.
Desirable Skills:
-
Previous experience as a Care Coordinator under CPA.

Trained in CBT, DBT, or other structured therapeutic modalities.

Access to own vehicle and full UK driving licence.
-
Prior work in CMHT, CMHRS, or related community roles.
