Key Responsibilities:

Caseload Management

  • Act as Care Coordinator under the Care Programme Approach (CPA), leading care planning and monitoring recovery outcomes.

Clinical Assessment & Intervention

  • Conduct comprehensive biopsychosocial assessments and deliver personalised interventions in line with NICE guidelines.

  • Support clients in achieving stability, independence, and improved mental health.

Risk & Crisis Management

  • Perform risk assessments, formulate risk management plans, and respond to acute episodes, including safeguarding concerns and mental health crises.

  • Engage in home visits and urgent clinical interventions as required.

Multi-Disciplinary Collaboration

  • Work in partnership with psychiatrists, social workers, psychologists, and third-sector organisations to deliver integrated support.

Medication Oversight

  • Support medication adherence, administer depot injections, monitor side effects, and offer psychoeducation around pharmacological treatments.

Documentation & Compliance

  • Maintain accurate, timely records using the Trust’s electronic patient record system.

  • Participate in clinical audits, service improvement meetings, and reflective supervision.


Candidate Profile:

Essential Qualifications:

  • Registered Mental Health Nurse (RMN) with active NMC registration.

Core Experience & Skills:

  • Proven experience in adult community mental health or recovery-based services.

  • Strong clinical decision-making and risk assessment skills.

  • Familiarity with Care Act, Mental Capacity Act, Mental Health Act, and safeguarding legislation.

  • Capable of lone working in community and home settings.

Desirable Skills:

  • Previous experience as a Care Coordinator under CPA.

  • Trained in CBT, DBT, or other structured therapeutic modalities.

  • Access to own vehicle and full UK driving licence.

  • Prior work in CMHT, CMHRS, or related community roles.

  • Excellent verbal and written communication.
