- Support individuals in the Preparing for Adulthood Pathway (18-25) to enhance their independence and well-being.
- Collaborate closely with health disciplines and other professionals to ensure a holistic approach to care.
- Develop and implement care plans tailored to individual needs, promoting independence and community involvement.
- Engage with families and carers, providing support and information to assist them in their roles.
- Contribute to strategic service development and improvement, ensuring high standards of practice.
