Purpose of the job:
To provide safe care and support for adults ‘at risk,’ ensuring that all their basic needs identified in their support plans and risk assessments are thoroughly met. Delivering person-centred, quality care tailored to individual requirements, this role is integral in assuring that everyone is treated with dignity, respect, and compassion.

What you will do:

  • Support the delivery of agreed plans of care, ensuring service users are safely and effectively supported.
  • Assist clients with personal care needs such as dressing, washing, toileting, and medication administration as outlined in their care plan.
  • Monitor the progress of service users and respond appropriately to changes or challenges, escalating issues to a manager when necessary.
  • Support individuals through emotional challenges and engage in developing activities that contribute positively to their well-being.
  • Accurately administer and record medication in compliance with relevant policies.
  • Maintain detailed and secure records of care plans and risk assessments, adhering to data protection guidelines.
  • Communicate effectively with service users, families, and health professionals to ensure a collaborative approach to care.
  • Ensure the safe use of equipment and support service users with mobility issues, conducting safety checks as required.
  • Offer encouragement and support to service users, motivating them to engage in daily activities.
  • Highlight and report any health and safety concerns to ensure a secure environment.
