What you will do:

- Serve as the first point of contact for adults in Hackney needing vital intervention, advice, and signposting from Adult Social Care.
- Screen and process all incoming referrals and communications effectively and efficiently.
- Address urgent needs, risks, and emergencies for ongoing cases, conducting unplanned reviews as necessary.
- Conduct Care Act assessments for new referrals that require thorough evaluation, working closely with residents and their carers in the community.
- Collaborate with Senior Social Workers, Review Officers, and Information Officers to deliver comprehensive support tailored to each situation.
