What you will do:

• Assess patients' physical and mental capabilities to develop tailored treatment plans.
• Implement therapeutic interventions aimed at improving patients' functional abilities.
• Collaborate with multidisciplinary teams, including speech therapists and case managers, to ensure comprehensive patient care.
• Monitor patients' progress and adjust treatment plans as necessary.
• Educate patients and their families on therapeutic techniques and exercises to promote independence.
• Maintain accurate documentation of patient evaluations, treatment plans, and progress notes.
• Stay updated with advancements in occupational therapy practices and medical imaging techniques.
• Conduct assessments to help Hampshire residents access their homes safely and independently, managing expectations for those who might have waited for service.
• Bring your experience with adults' services and PD clients, along with adept moving and handling skills.
• Travel efficiently with a full UK driving licence and a vehicle insured for business use, ensuring seamless visits to residents.
• Meet the requirement of a minimum 2 years of practical experience and hold the necessary degree and HCPC registration.
