What you will do:
- Undertake comprehensive assessments of children with disabilities, ensuring all practices are child-focused, holistic, and evidence-based under the Children Act 1989 and 2004.
- Identify and carefully manage risks, safeguarding children appropriately with timely and proportionate interventions.
- Develop and implement tailored child-in-need, child protection, and care plans, addressing the unique needs of disabled children and their families.
- Collaborate with parents, carers, health and education professionals to drive multi-agency, outcome-focused planning.
- Fulfil statutory duties, including conducting Section 17 and Section 47 assessments, managing Looked After Children (LAC) reviews, and participating in court proceedings as needed.
- Maintain precise and prompt case recording using the electronic case management system.
- Actively participate in and contribute to core groups, CIN meetings, CP conferences, and statutory reviews.
- Provide essential support to disabled children transitioning into adulthood to ensure continuity of care, collaborating seamlessly with adult services.
- Advocate for inclusion, independence, and access to community resources for children with disabilities and their families.
- Engage in supervision, team meetings, training, and continuous professional development to enhance your skills and service delivery.
