Job Purpose:

  • The postholder will ensure the effective use of medicines that meets the needs of patients, developing strong relationships with stakeholders in accordance with the Partnership of Purpose.
  • You will champion deprescribing efforts, work on reducing polypharmacy, and lead initiatives to foster seamless care transitions between primary and secondary settings.


What You Will Do:

  • Lead deprescribing initiatives and devise evidence-based strategies to lessen polypharmacy.
  • Improve interface prescribing by identifying solutions for cost savings, reducing errors, and enhancing quality using national and local guidance.
  • Design and conduct audits, create reports, and provide training based on findings.
  • Serve as the primary interface between Pharmacy and various healthcare entities, monitoring best practices and implementing legislative changes as needed.
  • Develop cross-sector collaborations, ensure shared care guidelines, and implement protocols.


Clinical Responsibilities:

  • Drive initiatives to reduce medication wastage and errors while enhancing prescribing quality.
  • Offer strategic advice on prescribing issues related to deprescribing and polypharmacy.
  • Supply clinical pharmacy services within hospital settings as required.


Leadership Duties:

  • Contribute to innovative service delivery and advise relevant committees on medicine usage and expenditure.
  • Manage and guide pharmacists and technicians.

Training and Research:

  • Engage in CPD, attend relevant courses, and provide feedback to peers.
  • Deliver training relating to medication management across various care teams.


Essential Skills and Experience:

  • Master's Degree in Pharmacy with General Pharmaceutical Council Registration.
  • Minimum three years of post-registration experience and verified CPD participation.
  • Proficient in medicines optimisation, change management, and clinical governance.
  • Strong communication and problem-solving abilities with a proactive approach.


Desirable Skills:

  • MRPharmS, independent prescriber status, management qualifications, and experience in business case writing.
  • Familiarity with multidisciplinary team work and networking skills.
