Role Purpose:

To provide professional social work services to adults with complex needs, ensuring assessments, safeguarding, and support planning are conducted in line with the Care Act 2014 and other relevant legislation. The role involves promoting autonomy, independence, and wellbeing while managing complex cases and working collaboratively with individuals, families, carers, and multi-agency partners.

What you will do:

- Conduct comprehensive, outcome-focused assessments and reviews of adults' social care needs, developing personalised support plans.
- Undertake safeguarding enquiries and investigations, ensuring informed decisions to protect individuals while promoting their rights.
- Develop, implement, and review support plans to maximise independence using an asset-based approach.
- Facilitate access to services, ensuring best value and compliance with legislation and policy.
- Manage a complex caseload independently, providing advice and guidance to colleagues and other agencies.
- Liaise with health, housing, and other sectors for coordinated and holistic support.
- Maintain accurate records and uphold professional standards and ethical frameworks.
- Assist with duty/intake functions, providing immediate professional guidance and arranging urgent support.
