What you will do:

- Conduct initial assessments for patients referred to the Local Mental Health Team.
- Document assessments and formulate suitable ongoing care plans.
- Make referrals to and liaise with other agencies to ensure comprehensive patient care.
- Identify any safeguarding concerns and escalate them appropriately.
- Travel within the community, requiring access to personal transport.

Skills Required:

Expertise in Adult Mental Health required for this role.
