What you will do:

- Conduct comprehensive assessments of patients' needs within the acute inpatient setting.
- Develop and implement tailored treatment plans to enhance patient recovery.
- Engage in effective discharge planning to facilitate seamless patient transitions.
- Provide and manage necessary equipment for patient care.
- Work autonomously to manage a diverse and dynamic caseload.

Requirements:

- Current and valid HCPC registration.
- Minimum of 2 years post-qualification experience in various inpatient settings such as medical, elderly, surgical wards, and the emergency department.
- Proficient in using System One.
- Up-to-date Moving & Handling training.
- Current Basic Life Support (BLS) certification.
- Strong IT skills for effective documentation and system navigation.
