We are currently seeking a skilled and compassionate Band 6 Occupational Therapist to join the Discharge Planning Team in Worthing. Your Key Responsibilities:

Conduct comprehensive assessments of patients’ functional and environmental needs.

Formulate, implement, and review individualised discharge plans that ensure safe, timely, and effective transitions from hospital to home.

Collaborate with healthcare professionals including nurses, physiotherapists, and social workers to deliver integrated patient care.

Provide patient and family education regarding adaptive strategies, equipment, and post-discharge care.

Maintain accurate clinical records and contribute to discharge planning meetings and case discussions.

Conduct community visits where necessary to assess home environments and support discharge readiness.
Requirements:

HCPC registration as an Occupational Therapist

Valid UK driving licence – essential for home/community-based visits

Previous experience in acute discharge planning or hospital settings is highly desirable

Strong communication, time management, and decision-making skills
