What you will do:

• Facilitate and optimise discharge planning for patients within general medical wards.
• Conduct comprehensive physiotherapy assessments to determine patient requirements for discharge.
• Develop personalised care plans, collaborating with multidisciplinary teams to ensure seamless transitions.
• Provide education and resources to patients and families for continued care post-discharge.
• Maintain accurate and timely documentation of patient progress and discharge planning in line with regulatory standards.

Requirements:

• Current registration with the Health and Care Professions Council (HCPC).
• Must hold a valid driving licence, as travel between locations within Chichester may be required to support patient needs.
