What you will do:
- Conduct clinical verification of prescribed medications, ensuring they meet safety and efficacy requirements.
- Provide expert clinical pharmacy advice to the multidisciplinary team to promote optimal patient outcomes.
- Counsel patients and their families or carers on medication use, side effects, and adherence to therapy plans.
- Support the supply and management of medicines for patients preparing for leave or discharge.
- Contribute to the development of evidence-based treatment plans to facilitate timely discharges.
- Actively participate in ward rounds and multidisciplinary meetings as an integral part of patient care.
- Ensure thorough compliance with all legal, professional, and organisational policies and procedures.

Responsibilities/General Duties:
