What you will do:
- Conduct a wide range of Care Act assessments, including carers’ assessments and Section 42 enquiries, adhering to necessary statutory requirements, and identifying risks, needs, and options.
- Undertake mental capacity assessments for individuals suspected of lacking the capacity to make specific decisions.
- Identify service users eligible for continuing healthcare or Section 117 funding, complete checklists, and collaborate with other professionals to deliver a comprehensive multi-disciplinary assessment.
- Execute Human Rights Act assessments for individuals with no recourse to public funds but requiring support under the Care Act.
- Carry out urgent reviews or annual reviews, including carers’ reviews in accordance with the Care Act as required.
- Manage a personal caseload, regularly monitor, review, and assess changes in client needs, ensuring support plans remain effective.
- Intervene swiftly in emergency scenarios to protect vulnerable adults, initiating necessary statutory actions.
- Collaborate effectively within the multi-disciplinary team and with colleagues in the West Locality, contributing to team meetings, forums, and service development initiatives.
- Develop care and support plans using Person-Centred and strengths-based approaches to deliver high-quality care while ensuring efficient use of the social care budget.
- Direct clients to community resources that meet their needs.
