What You Will Do

• Manage a caseload of service users with urgent and complex needs.

• Undertake Occupational Therapy specialist assessments in clients’ homes to determine eligibility and risk.

• Develop tailored, outcome-focused support plans to promote independence and prevent hospital admissions.

• Recommend and arrange for the provision of equipment and home adaptations, including major adaptations where required.

• Work collaboratively with social workers, care providers, and other allied professionals to ensure coordinated service delivery.

• Maintain accurate case records in line with organisational policies.

Requirements

• HCPC registration as an Occupational Therapist.

• UK-based OT experience within Adult Social Care or Local Authority settings.

• Full UK driving licence and access to a vehicle (home visits required across Ealing).

• Strong knowledge of relevant legislation (e.g., Care Act 2014, Housing Grants, and Disabled Facilities Grants).

• Ability to work independently and manage a busy caseload effectively.
