What you will do:

• Provide clinical pharmacy services under the guidance of the Advanced Clinical Pharmacist to both inpatient and community services across various specialties.
• Actively participate in ward rounds and multidisciplinary team meetings, taking responsibility for medicines reconciliation upon patient admission and optimizing medication management throughout their stay.
• Engage in pharmacy authorization of discharge summaries, ensuring a seamless transition of care.
• Be prepared to travel between several service sites, requiring a valid driving license.
