What you will do:

• Conduct advanced assessments of mainly frail patients to design effective rehabilitation plans focusing on balance, strengthening, mobility progression, and discharge planning.
• Collaborate with and support Band 5 Physiotherapists, ensuring their development and confidence in handling patients.
• Efficiently delegate appropriate tasks to therapy technicians, ensuring optimal care delivery.
• Work closely with a multidisciplinary team, contributing your expertise to create comprehensive treatment plans.
• Utilise your experience in managing frailty and rehabilitation cases to provide exceptional patient care.
• Must have an HCPC registration and a valid driving licence.
