Social Worker – Assessment Team

Role Overview:

Key Responsibilities:

Carry out comprehensive assessments of individuals and families to understand their needs, risks, and strengths.

Develop and implement recovery-focused and strengths-based care plans in collaboration with service users and their support networks.

Work in partnership with multi-agency professionals to deliver integrated and person-centred support.

Attend and contribute to case conferences and statutory reviews, producing clear and analytical reports.

Ensure accurate and timely case recording, complying with local procedures and national standards.
Requirements:

Qualified Social Worker with current registration with Social Work England.

Previous experience in assessment and safeguarding within children’s or adult services (as relevant).

Strong communication, report-writing, and risk assessment skills.

Ability to work independently and collaboratively within a multidisciplinary team.
