Key Responsibilities:

• Conduct comprehensive functional assessments for adults with physical, sensory, or cognitive needs.

• Create and implement individualised care plans that promote independence and quality of life.

• Undertake manual handling risk assessments, ensuring safety in the home and community settings.

• Prescribe and monitor the use of simple and complex equipment, and recommend adaptations to living environments.

• Evaluate housing needs and contribute to void assessments and major adaptation planning.

• Maintain accurate records and ensure compliance with the Care Act, Equality Act, and other relevant legislation.

• Engage with service users, carers, families, and a multidisciplinary network to coordinate services effectively.

Candidate Requirements:

• HCPC-registered Occupational Therapist

• Minimum 2 years' experience in a local authority or social services setting

• Confident in handling a complex caseload involving adaptations, manual handling, and equipment provision

• Valid UK driving licence and access to a vehicle (community travel required)
