What you will do:

• Conduct comprehensive social care assessments, creating effective support plans for individuals and ensuring that they receive appropriate reviews.
• Implement safeguarding measures through detailed risk assessments, protection planning, and participation in meetings and reviews.
• Facilitate mental capacity assessments and best interest decisions, including applications to the Court of Protection.
• Perform carers assessments and reviews, supporting those who care for others by identifying their needs.
• Engage with NHS checklist and DST meetings, ensuring that healthcare and community services are integrated efficiently.
