Assessment & Planning: Carry out initial and ongoing assessments, co-producing person-centred recovery plans with clients. Group Facilitation: Design and deliver therapeutic groups/workshops focusing on relapse prevention, life skills, and behavioural change. Evidence-Based Interventions: Apply techniques such as Motivational Interviewing, CBT, and harm reduction strategies. Multi-Agency Coordination: Liaise effectively with health services, housing support, criminal justice partners, and community services. Safeguarding & Risk Management: Identify safeguarding concerns, escalate appropriately, and respond to high-risk behaviour with coordinated support. Compliance & Documentation: Maintain accurate case records, ensuring service delivery meets contractual and legal standards. Lived Experience Integration: Where applicable, use peer support principles or personal insight to build rapport and trust with service users. Essential Requirements Experience: Proven background supporting individuals affected by substance misuse, offending behaviour, or co-occurring mental health issues. Qualifications: NVQ Level 3 in Health & Social Care (or equivalent) preferred. Skills: Strong in risk assessment, recovery planning, and delivering group/1:1 interventions. Compliance: Enhanced DBS clearance (Adult Workforce), valid within the past 12 months or on the Update Service.
