An exciting opportunity has arisen for an experienced Social Worker to join the Learning Disability Adult Social Care Team in Wandsworth. Key Responsibilities

As a Social Worker within the Learning Disability Adult Social Care Team, you will:

• Undertake assessments of adults with learning disabilities and their carers, identifying needs, risks, and strengths.

• Develop and review person-centred care and support plans that promote independence, choice, and wellbeing.

• Apply social work values and professional judgement to safeguard vulnerable adults and manage risk effectively.

• Work in accordance with relevant legislation and guidance, including the Care Act 2014 and adult safeguarding frameworks.

• Make timely and proportionate decisions, including responding effectively to urgent and emergency situations.

• Work collaboratively with service users, carers, families, and multi-agency partners to deliver coordinated support.

• Maintain accurate, timely, and high-quality case records in line with statutory and local authority requirements.

• Contribute positively to team meetings, supervision, and continuous service improvement.

Requirements

• Degree or equivalent qualification in Social Work

• Registration with Social Work England

• Post-ASYE experience within adult social care

• Experience working with adults with learning disabilities

• Strong knowledge of adult safeguarding, person-centred practice, and strengths-based approaches
