About the Role
An exciting opportunity has arisen for an experienced Social Worker to join the Learning Disability Adult Social Care Team in Wandsworth. Key Responsibilities
As a Social Worker within the Learning Disability Adult Social Care Team, you will:

Undertake assessments of adults with learning disabilities and their carers, identifying needs, risks, and strengths.

Develop and review person-centred care and support plans that promote independence, choice, and wellbeing.

Apply social work values and professional judgement to safeguard vulnerable adults and manage risk effectively.

Work in accordance with relevant legislation and guidance, including the Care Act 2014 and adult safeguarding frameworks.

Make timely and proportionate decisions, including responding effectively to urgent and emergency situations.

Work collaboratively with service users, carers, families, and multi-agency partners to deliver coordinated support.

Maintain accurate, timely, and high-quality case records in line with statutory and local authority requirements.

Contribute positively to team meetings, supervision, and continuous service improvement.
Requirements

Degree or equivalent qualification in Social Work

Registration with Social Work England
-
Post-ASYE experience within adult social care
-
Experience working with adults with learning disabilities
-
Strong knowledge of adult safeguarding, person-centred practice, and strengths-based approaches
