What you will do

  • Conduct high-quality assessments and reviews, including annual reviews and progression reviews, to identify individual needs and aspirations.
  • Develop and implement strengths-based, outcome-focused care and support plans that promote independence, resilience, and community inclusion.
  • Coordinate timely interventions and support planning at key transition points.
  • Work collaboratively with health professionals, carers, families, advocates, and partner agencies to deliver coordinated support.
  • Support adults with learning disabilities, autism, and complex needs to achieve positive outcomes and reduce long-term dependency.
  • Promote choice, control, independence, dignity, and wellbeing in all aspects of practice.
  • Undertake Care Act assessments and reviews in accordance with relevant legislation and local procedures.
  • Manage a varied caseload while maintaining accurate and timely case records.
  • Participate in safeguarding processes and contribute to risk assessment and risk management activities.
  • Engage in regular supervision, reflective practice, and continuous professional development.
  • Contribute to service improvements and the development of innovative approaches to supporting adults with learning disabilities.
