What you will do:

Assess the needs of individuals and families affected by domestic abuse and identify appropriate support pathways.
Develop and implement personalised support plans in partnership with service users and relevant agencies.
Work collaboratively with community partners and multidisciplinary teams to improve outcomes.
Advocate for clients’ rights and coordinate multi-agency meetings to ensure effective support.
Provide emotional support, practical guidance, and safety planning to empower clients.
Maintain accurate and up-to-date case records in line with statutory requirements and organisational policies.
Participate in team meetings and contribute to the continuous improvement of services.
