What you will do:

Manage a caseload of children with disabilities, undertaking critical statutory assessments under Section 17 and Section 47 of the Children Act 1989.

Develop and implement comprehensive Child in Need, Child Protection, and Looked After Children plans that focus on each child's wellbeing, developmental, health, and social care needs.

Conduct holistic assessments, including short breaks, direct payments, and specialist support packages aligned with local eligibility criteria.

Collaborate closely with multi-agency partners such as paediatricians, therapists, and Special Education Needs services, ensuring seamless transition planning with adult services.

Lead safeguarding enquiries and risk assessments, providing essential protection and intervention for vulnerable children, ensuring their safety and welfare.

Facilitate transition planning into adulthood, coordinating with adult social care and other services to ensure smooth continuity of care.
Person Specification:
Qualifications:
-
Hold a degree in Social Work (BA/MA or equivalent)
-
Completion of ASYE or relevant post-qualifying experience in children’s services
Registration & Compliance:
-
Must be registered with Social Work England
-
Possess extensive knowledge of the Children Act 1989/2004, Children and Families Act 2014 (SEND), and safeguarding standards
Key Skills:
-
Skilled in undertaking complex, holistic assessments for children with disabilities
-
Strong safeguarding and risk management capabilities
-
Proficient in SEND processes, such as Education, Health and Care Plans (EHCPs)
Experience:
-
Proven experience working with children with disabilities or within a specialist CWD team

Experience managing complex cases involving health, behavioural, and social care needs
