Social Worker - Central Adults Community Team (Shropshire) - £28.40 per hour - Full-time, Ongoing Contract

What you will do:
- Conduct comprehensive face-to-face assessments of individuals' needs in the Central Shropshire community.
- Develop and implement personalised care plans that promote wellbeing and independence.
- Coordinate with multidisciplinary teams to provide holistic support tailored to each service user's requirements.
- Advise and assist service users and their families, directing them to appropriate services and resources.
- Ensure compliance with legal and organisational policies, maintaining accurate and up-to-date records.
Requirements of the Social Worker:
- Hold a degree or equivalent in social work with current registration with Social Work England.
- Possess significant frontline experience in Social Services within the UK as a qualified Social Worker.
- Demonstrate excellent interpersonal skills and the ability to adapt to various situations effectively.
- Be a licensed driver to facilitate travel for assessments across Central Shropshire.
