What you will do:

- Engage with adults in the community to assess and address their needs.
- Develop and implement support plans aimed at promoting independent living.
- Coordinate with other professionals to deliver comprehensive care solutions.
- Undertake visits and direct work with customers who have diverse needs, acting as a key player in risk management and safeguarding processes.
- Perform carers' assessments and foster strong relationships with family members to support seamless care delivery.
- Use your expertise to contribute to the care of individuals with conditions such as dementia, substance misuse issues, and homelessness, ensuring tailored support.
- Ensure compliance with legal and regulatory requirements within adult social care.

Candidate requirements:

- Be a qualified social worker with more than three years of experience in adult social care.
- Must be able to be office-based at least once a week and conduct visits on other days.
- A car driver with access to a vehicle.
- Possess experience in dealing with dementia, substance misuse, homelessness, hoarding, safeguarding adults (S42 inquiries), and carers assessment.
