Social Worker locum position within the Adults Team in Hertfordshire, earning £32 hourly.

What you will do:

  • Conduct comprehensive assessments of adults with care and support needs, focusing on strengths-based approaches in line with the Care Act 2014.
  • Design, implement, and regularly review tailored care and support plans to enhance independence, wellbeing, and achieve positive outcomes for your clients.
  • Manage a diverse caseload, addressing complex issues that arise with individuals who have physical disabilities, mental health needs, are older, or are considered vulnerable.
  • Undertake safeguarding enquiries, assess risks, and proactively work to protect adults from abuse or neglect.
  • Carry out Mental Capacity Assessments and actively participate in decision-making processes to ensure the best interests of clients are prioritized.
  • Collaborate effectively with families, carers, healthcare professionals, and other multi-agency partners to deliver cohesive support services.
  • Organise and oversee suitable care packages and services, ensuring they meet client needs efficiently and effectively.
  • Maintain detailed and accurate case records, assessments, and professional reports in compliance with statutory and organisational standards.
