What you will do

• Conduct holistic assessments, develop care plans, and evaluate patient care within their homes and community settings.
• Deliver diverse clinical interventions, including wound management, catheter care, medication administration, and care for palliative and end-of-life patients.
• Efficiently manage and prioritise an assigned caseload, swiftly responding to evolving clinical needs.
• Participate in community response and admission avoidance initiatives to help patients remain safely at home.
• Provide some travel support to sites around the Haslemere, Guildford, and Waverley areas when needed.

Person Specification

• Registered Nurse (Adult) qualification with an active Nursing and Midwifery Council (NMC) registration.
• District Nursing qualification or a specialist community qualification is desirable for Band 6 roles.
• For Band 5 roles, practical experience through pre-registration placements or post-registration nursing is essential.
• For Band 6 roles, significant post-registration experience within the community or district nursing is required.
• Must Be A Car Driver - Must be able to drive to sites around the Haslemere/Guildford/Waverley area
