What you will do:

Collaborate with the hospital discharge team and Integrated Care Board to facilitate timely and safe discharges for those with NHS fast track funding.
Conduct Care Act Assessments, risk assessments, and continuing health care checklists to ensure individuals receive appropriate care and support in the community and local hospices.
Engage in safeguarding enquiries under Section 42 of the Care Act, ensuring that the rights and safety of individuals are protected.
Work with individuals, families, and carers in both hospital and community settings to create comprehensive support plans considering Mental Capacity and Human Rights legislation.
Coordinate Decision Support Tool meetings to evaluate and plan for continuing health care needs.
