As a Social Worker within the Adults Neighbourhood Team, you will work with adults aged 18 and over who have care and support needs resulting from physical disability, learning disability, mental ill health, sensory impairment, frailty, or long-term health conditions. You will deliver high-quality, person-centred social work that promotes independence, wellbeing, and positive outcomes, while ensuring adults are safeguarded from abuse and neglect.

What you will do:

  • Assessment and Care Planning: Undertake comprehensive Care Act 2014 assessments to identify eligible care and support needs. Develop, implement, and review outcome-focused care and support plans while promoting community resources, preventative services, and informal support networks.

  • Safeguarding Adults: Identify adults at risk and carry out safeguarding enquiries. Manage risks and support individuals to make informed decisions, participating in safeguarding meetings and applying Making Safeguarding Personal principles.

  • Case Management: Manage a diverse caseload with timely assessments, reviews, and interventions. Maintain accurate case records and prepare reports for legal proceedings and professional meetings.

  • Multi-Agency Working: Collaborate with health professionals, housing services, voluntary sector partners, and other agencies to coordinate effective support for adults and carers.

  • Mental Capacity and Best Interests: Conduct Mental Capacity Act assessments and facilitate Best Interests decision-making processes.

  • Supporting Carers: Undertake carers' assessments and provide access to appropriate support services, recognising the vital role unpaid carers play.

  • Professional Practice: Uphold professional standards through regular supervision, reflective practice, and ongoing professional development.
