What you will do:

Conduct comprehensive assessments for adults transitioning from hospital settings into appropriate care and support arrangements.

Undertake Discharge to Assess (D2A) assessments and contribute to timely and effective discharge planning.

Work collaboratively with health colleagues, social care professionals, and partner agencies to coordinate discharge pathways.

Provide specialist advice and guidance regarding Continuing Healthcare (CHC) processes, assessments, and support planning.

Develop person-centred care and support plans tailored to individual needs, outcomes, and preferences.

Engage with service users, carers, and families to ensure their views and wishes are central to decision-making.

Maintain accurate, timely, and compliant case records in accordance with organisational policies and statutory requirements.

Attend multidisciplinary meetings and case reviews to support coordinated and effective care planning.

Monitor and review care arrangements, ensuring support remains appropriate and outcome focused.

Apply relevant legislation, including the Care Act 2014, Mental Capacity Act 2005, and safeguarding frameworks within everyday practice.
