What you will do:

• Conduct comprehensive assessments for adults transitioning from hospital settings into appropriate care and support arrangements.

• Undertake Discharge to Assess (D2A) assessments and contribute to timely and effective discharge planning.

• Work collaboratively with health colleagues, social care professionals, and partner agencies to coordinate discharge pathways.

• Provide specialist advice and guidance regarding Continuing Healthcare (CHC) processes, assessments, and support planning.

• Develop person-centred care and support plans tailored to individual needs, outcomes, and preferences.

• Engage with service users, carers, and families to ensure their views and wishes are central to decision-making.

• Maintain accurate, timely, and compliant case records in accordance with organisational policies and statutory requirements.

• Attend multidisciplinary meetings and case reviews to support coordinated and effective care planning.

• Monitor and review care arrangements, ensuring support remains appropriate and outcome focused.

• Apply relevant legislation, including the Care Act 2014, Mental Capacity Act 2005, and safeguarding frameworks within everyday practice.
