What you will do:

- Conduct strength-based Care Act assessments to support personalised care planning
- Facilitate and support Continuing Healthcare (CHC) processes to ensure comprehensive care delivery
- Develop and review person-centred support plans, ensuring that clients' needs and preferences are at the heart of all decisions
- Engage in safeguarding work in compliance with the Care Act 2014, protecting vulnerable adults from harm
- Collaborate within a multi-disciplinary team environment, sharing insights and strategies to enhance client outcomes
- Apply the Mental Capacity Act 2005 in practice to ensure informed and voluntary decision-making by clients

To be considered, you will need:

- Registration with Social Work England
- A relevant Social Work qualification
- Experience in Adult Social Care with proven expertise in completing Care Act assessments
